Umang Communtiy Care Centre

 
 

GSS has established Umang Community Care Centre to provide care and support to PLHA. The goal of Umang CCC is to reduce HIV related morbidity and mortality in adults and children and to mitigate the impact of HIV on children and women headed households.

Umang CCC has been set up with support from Population Foundation of India. Other stakeholders are Indian Network for People Living with HIV/AIDS (INP+), Hindustan Latex Family Planning Promotion Trust and Catholic Bishops Conference of India.

Community Care Centre is a national programme. It is a place where a PLHA receives care and support with out-patient and in-patient treatment facilities. It has multi-dimensional roles defined for it:

In-patient

  • All PLHAs started on ART (at the ART centre), will be sent to the CCC for a minimum of 5 days of inpatient care
  • Medical monitoring for side effects and tolerance to ART regimen
  • Verification of patient address
  • Follow up home visits
  • Preparing family and/spouse for acceptance of test status
  • Counselling support, drug adherence
  • Treatment of OIs
  • Routine regular monitoring of CD4 testing and other medical concerns
  • psychosocial support and counselling
  • Treatment literacy
  • Positive prevention
  • Referrals

Out Patient

  • Home based care including counselling an outreach facility
  • ICTC facility, including HIV testing and PPTCT services
  • DOTS facility
  • Condom distribution

Referrals to and from CCC

  • ICTC and PPTCT
  • ART Centre, Paediatric HIV services/ART centre for CLHA
  • Medical facilities for specialised services as required
  • DOTS for treatment for TB
  • Other government hospitals and private hospitals
  • Drop -in centres
  • Home based organisations
  • Local PLHA networks (DLNs)
  • Rehabilitation centres
  • Other services as required
